What Most Doctors Don’t Check When You Say You’re Always Tired
Told your labs are normal, but still exhausted? Discover the key markers most doctors overlook what your fatigue is really trying to tell you.
You’ve said it before. Maybe more than once. “I’m just exhausted all the time.”
Chances are, the response you got was: get more sleep, reduce stress, maybe try exercise. Blood work comes back “normal.” You leave the appointment feeling dismissed because you know something is off, even if the numbers say otherwise.
Here’s the truth: standard lab panels miss a lot. And fatigue is one of the most underinvestigated symptoms in conventional medicine.
“Normal” Doesn’t Always Mean Optimal
This is one of the most important distinctions in functional and integrative medicine.
Standard blood panels are designed to rule out disease, not to identify the gap between feeling sick and feeling your best. Reference ranges are based on population averages, so you can fall within “normal” and still be running on empty.
A TSH technically in range but at the high end. Ferritin levels low enough to cause fatigue but not low enough to flag anaemia. Vitamin D not deficient on paper but far below the level your body needs to function well.
Normal isn’t the goal. Optimal is.
What Most Doctors Don’t Check
1. Ferritin (Not Just Iron)
Most panels check serum iron. Ferritin, the protein that stores iron in your cells, is a more accurate marker of how much iron your body actually has available. Low ferritin is a common and overlooked cause of chronic fatigue, brain fog, and poor exercise recovery. You don’t need to be anaemic to feel the effects.
2. Full Thyroid Panel
A standard TSH test tells you little about how your thyroid is functioning. A complete thyroid panel includes Free T3, Free T4, Reverse T3, and thyroid antibodies. Without these, conditions like subclinical hypothyroidism or Hashimoto’s thyroiditis can go undetected for years while you’re told everything looks fine.
3. Vitamin D (25-OH)
Vitamin D isn’t just a vitamin; it functions more like a hormone, influencing energy, mood, immune function, and inflammation. Deficiency is common, especially in people who work indoors or live in low-sunlight climates. Yet it’s frequently left off routine panels. Levels below 40 ng/mL can significantly impact how you feel day to day.
4. Cortisol and Adrenal Function
Cortisol is your primary stress hormone. When dysregulated, either chronically elevated or crashed from long-term burnout, it causes persistent fatigue that sleep doesn’t fix. A single morning cortisol draw gives limited information. Serum magnesium is almost always normal, even when you’re deficient. Your body pulls magnesium from cells and bones to maintain stable blood levels. A red blood cell (RBC) magnesium test measures intracellular levels, where it matters. Magnesium is involved in over 300 enzymatic processes, including energy production, sleep regulation, and muscle recovery. Low levels are more common than most people realise.
6. Fasting Insulin and Blood Sugar Patterns
Glucose on a standard panel only shows a snapshot. It doesn’t reveal how your body manages blood sugar throughout the day. Chronically elevated insulin, even with “normal” fasting glucose, can cause energy crashes, afternoon fatigue, sugar cravings, and difficulty concentrating. An HOMA-IR calculation or continuous glucose monitoring provides a more useful picture. W-grade inflammation is one of the most common drivers of persistent fatigue and one of the least tested. Markers like high-sensitivity CRP (hs-CRP), homocysteine, and interleukin-6 can reveal inflammation that’s quietly draining your energy and affecting your brain, joints, and metabolism without obvious symptoms.
8. Nutrient Levels: B12 and Folate
B12 deficiency is common, especially in people who take medications like metformin or proton pump inhibitors, follow a plant-based diet, or have digestive issues affecting absorption. Low B12 affects nerve function, red blood cell production, and energy metabolism. Folate works alongside B12 and is equally important, especially for those with MTHFR gene variants that affect nutrient processing.
Fatigue Is a Symptom, Not a Diagnosis
This is worth saying clearly: being tired all the time is not normal. It is not an inevitable part of ageing. It is not something you can manage with more caffeine and earlier bedtimes.
Fatigue is your body communicating. It’s a downstream effect of something upstream that hasn’t yet been identified.
When that upstream cause goes unaddressed, whether a nutrient deficiency, sluggish thyroid, dysregulated cortisol, or chronic inflammation, no amount of rest, willpower, or lifestyle tweaks will fully resolve it.
The Right Questions Lead to the Right Answers
If you’ve been told your labs are normal but still don’t feel like yourself, the problem may not be you but what’s being tested.
A root-cause approach looks beyond the standard panel. It asks not just “Is anything critically wrong?” but “What does this person need to feel and function at their best?”
That shift in question changes everything.
